Intradiploic epidermoid cysts are relatively rare tumors that arise between the 2 tables of cranial bones. We report a very rare case of intradiploic epidermoid cyst of the maxilla, which was managed through dual endoscopic approach. We recommend the dual endoscopic approach, maxillary anterior wall trephination, and middle meatal antrostomy for removal of intradiploic epidermoid cyst of the maxilla. The endoscopic approach through maxillary anterior wall trephination and middle meatal antrostomy allows for good visualization, removal of the intradiploic epidermoid cyst, prompt spontaneous drainage, avoidance of the damage of the nasolacrimal duct and inferior turbinate, and good cosmetic effect.
